13.1 Spaghetti Wrist


The spaghetti wrist can involve division of up to 16 structures (Fig. 13‑1). These injuries are commonly secondary to glass or knife lacerations and also to deliberate self-harm. A systematic approach is needed to avoid secondary complications and to optimize the efficiency of surgical repair (Fig. 13‑2, Fig. 13‑3, Fig. 13‑4, Fig. 13‑5, Fig. 13‑6, Fig. 13‑7, Fig. 13‑8, Fig. 13‑9, Fig. 13‑10, Fig. 13‑11, Fig. 13‑12, Fig. 13‑13).

Glass injuries are often more extensive than you think. One must perform a detailed preoperative examination so that you can identify which structures are likely to be damaged and which ones you must identify. One should start the dissection outside the zone of trauma to help identify normal anatomy, and then follow normal structures into the zone of trauma. This avoids ineffective dissection around the wound.
