A 45-year-old African American man presents to the dermatology clinic for evaluation of painful recurrent bumps on the back of his neck. He began developing painful, pus-filled bumps on the back of his scalp and neck in his early 30s and some have healed with scarring. On examination, there are numerous firm follicular-based papules and keloidal plaques on the occipital scalp with associated areas of alopecia (18).
