A 19-year-old African American woman presents to the dermatology clinic with textural changes and darkly pigmented areas on her neck, elbows and hands. She has no known medical history but her body mass index is 28.5. She reports that she has tried numerous over-the-counter creams and bleaching agents to try to even out the texture and colour, with no improvement. On examination, there are velvety plaques overlying the knuckles and proximal interphalangeal joints of the hands (38), as well as the elbows and the base of the neck.
