Smith, Jr., MD Evidence Ranking D Expert Rating 2 Abstract Background Traditional concern for the use of epinephrine in the fingers is founded on 21 cases of digital necrosis reported after injection with a local anesthetic with epinephrine. In all cases, which usually occurred before 1950, procaine was used, which may have been expired, giving it a toxically low pH. Phentolamine readily reverses the effects of epinephrine in the fingers. The evidence concerning links between the use of epinephrine and the occurrence of digital necrosis was reviewed.
