Abstract



Background


Inhalation injury causes several effects. Casts and proteinaceous materials accumulate in the airway, causing bronchoconstriction and airway hyperreactivity resembling what is seen in reactive airway diseases. If a large cutaneous burn accompanies the inhalation injury, capillary permeability in the lung and distal airway increases. The resulting pathologic condition is similar to what happens in acute lung injury (ALI), acute respiratory distress syndrome (ARDS), and reactive airway disease. Few therapies are effective in reactive airway disease and ARDS, but inhaled β2-agonists have shown efficacy in preclinical studies of ALI/ARDS. The properties of β-agonists; the clinical effectiveness of these agents in ALI/ARDS, reactive airway disease, and inhalation injury; and the efficacy of β2-agonists in inhalation injury were outlined.



Properties


β2-adrenoreceptors are found in airway smooth muscle and throughout the lung, especially in the smaller airways. Receptors occur in pulmonary epithelial and endothelial cells, type II cells, and mast cells. β2-agonists have a preferential affinity for β2-receptors and are designed to minimize the cardiac effects of using nonspecific β
